Thrown Weapons: Ballisteer/Projectilist
Axe: Axe Warrior / Skullsplitter
Dagger: Dagger / Slicer
Mace: Mace Warrior / Skullcrusher
Spear: Spear Warrior / Impaler
Staff: Staff Warrior / Master of Staves
Sword: Swordfighter / Duelist
Unarmed Combat: Unarmed Brawler / Pugilist
Alchemy: Apothecary / Alchemist
Cooking: Chef / Iron Chef
Fletching: Fletcher / Master Fletcher
War Magic: War Adept / Warlock
Life Magic: Life Adept / Theurgist
Item Enchantment: Item Adept / Artifex

You go to one of the naming tents. I know there's one in Eastham and Qalabar. Pick up the puzzle piece from the ground (can only do this once per day), buy the corresponding skill piece you want from the vendor (10k I think), but your skill and use the pieces on each other. If you combine them successfully, hand the completed puzzle to the namer and get your title.
