MANY thanks goes to Aldur (AC Tips), Oanda (AC Tip), Sintax (AC Tips), Roofus (COD), gith (COD) and multiple anonymous posters for their revisions and corrections. Here are ALL my notes that I've gathered from all over the place for this quest.
Do the Three Towers Quest and get the High Acolyte's skull. The quickest way to get to the Three Towers Quest is to start at the Web Maze. Take the Mayoi portal (49.4S,65.3E). Run east to the South Direlands (62.2S,77.1E). Put on all your protection and run due south to the beach, then directly east to the Direlands Tower @ (87.6S,12.3W). If you'd never done the Three Towers Quest here is a link to a lot of Three Towers Quest walk-throughs.
Once you have the High Acolyte's skull and the 4-use key, enter the portal. At this point you can recall, take the surface portal or try your luck at one of the four mystery chests that give 50/50 awesome loot (I gotten base AL185 pair of Gauntlets and an Acid V shirt with 190 dif before).
Correction: The Grievous Vault is NO longer necessary to the quest.
Once you have the High Acolyte's Skull, go to Le-Ai Rea (75.9S, 57.3E). The shortest route there, pending your LS in AB, is to take the Baishi Portal, run south to the South Shore Heights Portal (54.6S,62.7E), then run NE to Le-Ai Rea (75.9S,57.3E). Give him the High Acolyte's Skull. You will receive the gold key, a one-use glowing key to the "Beyond the Mines of Despair" portal and the 4 use key to the first door in the Mines as well as the 12000 XP you would normally receive from the Ivory Crafter in AB.
Once you finish with Le-Ai Rea run a little north to the Mines of Despair (75.4S, 57.3E). NOTE: You need at least 2 groups of people to activate the multiple levers on different levels. Also must be able to beat Lich Lords with a 2 minute spawn rate as well as a Lich Overseer. (Credits: Ryth-al-Rothgar) Of interest is the nearby Town of the Dead. How to pull the levers: The lever !@#$%^&* will need to get through the locked door at the beggining then jump left and down. All the other people/questers need to jump down to the right (near the wisp) and go to the unopenable door behind which a lich lord spawns.
The lever guy then proceeds past a skeleton captain and down past a few skel warriors to a large steel door. The lever !@#$%^&* must have access to lockpick as you cannot lag through the large steel mine door that is locked. The you need to go to the room with the 2 skel warriors, turn left, head north then right then run until you get to a door to a room with a button in it. (see http://www.lostdungeons.com for maps). This button then opens the door where the other people are waiting.
Then you jump down in the circular drop right next to the room with the first button. Proceed south to the room with the lich lord and the next button. This button will open up the door to the lilch overseer and crew.
Note: It is possible to have the lever person be able to do the quest, but they must tie to the outside portal. They push the first button, get to the second puch that then recall and run through then jump down and run through the two doors. It is just easier to station a mule at the 2 buttons though.
